1. High-Precision Focused Shockwave Therapy | f-ESWT
- Mechanism: Focused shockwaves deliver energy at a selected intensity precisely to areas where fibrotic plaque has accumulated.
- Clinical Effect: May help pain in some patients, but evidence for reducing plaque or curvature is uncertain and an outcome should not be guaranteed.
2. Mechanical Tension Therapy | Vacuum Erection Device
- Mechanism: A medical vacuum erection device applies a consistent mechanical stretch to the tissue.
- Clinical Effect: Helps prevent penile shortening and promotes blood-vessel expansion throughout the area, helping maintain shape and improve long-term connective-tissue flexibility.
3. Intralesional Regenerative Therapy & Fibrosis Release
- Mechanism: Ultrasound-guided targeted injections reduce tension within fibrotic tissue. The primary aim is to address plaque in the tunica albuginea and improve tissue flexibility.
- Modalities:
- Advanced AP-PRP: An autologous procedure for which evidence in Peyronie's disease remains limited; benefits, risks, and established alternatives require discussion.
